Social Prescribing Service
Social Prescribing is a way in which we can help patients to link with a range of local, non-medical activities, opportunities and support that can improve their health and wellbeing. Social Prescribers give patients time to focus on ‘What Matters to me’ and taking a holistic approach to health and well-being, by connecting patients to community groups for practical and emotional support. We support existing community groups to be accessible and sustainable, and work collaboratively with all local partners.

Community First's Home Services provides cost-effective assistance for anyone living in Gosport, Fareham or Havant, and surrounding areas who is unable to properly maintain their homes from their housework and/or shopping to fixing things, decorating and nurturing their garden! We operate a fleet of wheelchair accessible minibuses that can collect you from your home, at pre-arranged times, enabling you to access major supermarkets and services. On your return home, the drivers will assist with carrying any shopping if required.
